The mission of Prime Home Care LLC is the ensure that our patients are living their highest quality life by providing quality services, and compassionate care tailored to each patient’s unique needs and collaborating with the patient, health care staff, and patient’s family to achieve this. We train our staff to treat all our patients as they would treat their family members because we believe that once under our care,we are family.
We are certified by the State of Indiana and Medicaid to provide both Attendant Care Services, and Structured Family Care services in several counties in Indiana.
Attendant Care Services
Attendant care is a type of support service where a caregiver or attendant provides assistance with activities of daily living such as bathing, dressing, eating, and mobility for someone who has a disability or medical condition. The goal of attendant care is to help individuals maintain their independence and improve their quality of life.
Structured Family Care Services
Structured Family Caregiving (SFC) is a model of caregiving that provides financial and supportive services for family and non-family caregivers. It allows caregivers to care for seniors and those with Traumatic Brain Injury (TBI) disabilities who wish to remain at home, or in the home of an approved caregiver, rather than become institutionalized in a nursing home.
